1. Unfortunately, no, you can't have all locations displayed in the drop-down.
2. Since you have multiple locations, they will be offered as an option before the calendar is displayed. You can add the booking form on 3 different pages, to be location-specific with the shortcode [ameliabooking location=1] and it will include the location automatically and the calendar will be displayed first.
You can add this script to your theme's footer.php file, usually located in ../wp-content/themes/YOUR-CURRENT-THEME/footer.php and add the script on the bottom of that page:
It should be loading on all pages, as long as you keep using the current theme. If you change the theme, you'll have to add it to the footer.php file of that other theme.
