Is there any way to add custom length appointments to the system? The employee needs to be able to manually configure start and end time of an appointment in the back end occasionally

I am sorry to disappoint you, but unfortunately something like this is not possible with the plugin's built-in features.
You can only define the duration of the service, and that determines how long the employees assigned to this service will work on the service.

Is there any way of setting services that only staff can set on the back end and customers can't see on the front end?

Sure, when you open a service, just disable "Show service on site" slider:
When you enable this, the customers won't see it, but your staff would be able to create appointments in back-end.
