If this happened once and since it happened not only for Amelia but with that other shop (plugin or whatever it is that you use notifications for as well for the appointment) this can be because of some server moment glitch or similar. If it happens more often and for some type of appointments always (or some type of notifications), please let us know so we can investigate why is it happening.
2. This is how Amelia works - when an appointment is created customers can still book if there are slots left on the front-end, but if you want to add a customer in the back-end, you don't create a new appointment but 'Edit' the already created one and just add the new customer to the list.
