I have added the cancellation link into the confirmation email. Works great but I have a question concerning refunds through paypal. If the client cancel the appointment, does he/she get automatically a refund via paypal or do I have to refund manually?

Unfortunately, the plugin cannot automatically process the refund, so you will need to do it manually.
