no changes have been made to the site but the booking form no longer loads it just spins blue circle.

This can't happen just like that, as you said by itself, in Amelia.
Could you please tell me have you changed something in your database, that maybe affected this? Or changed the work hours of this employee so the services' duration won't fit anymore in their work hours?
