I'm not sure I understand what you mean?
If you want to add special hours which do not apply to the standard working hours, you can add special days to each employee:
My advice would be to add a new service as well - the one which will not appear in the front-end booking form:
This way, employees can create new appointments at predefined special times, for the service which is not displayed on site.

Internally should be posible to have an appoinment outside the established working hours because my doctors need to make exceptions. how can i do? Can you considere it to future changes?

Like I already mentioned - if you set up the employees not to work after a specific time - you cannot add exceptions. The workaround I told you about is the only way of doing it - creating a new service, not showing it on site, and adding special days, will allow you to create custom appointments for your employees.

Yes the case use is that for example a doctor have his working hours but in some case need to make attention outside the established working hours. In others plataforms like vcita.com it´s very useful feature.

I assure you, it's not as weird as you think. Imagine being a doctor or coach, schedules are not always respected.
With Amelia, this exception cannot be handled, so the insertion of appointments is limited.
