You don't need it in the Custom JS. Custom JS and CSS is just the name of the tab in main settings.
Just leave it in Custom CSS, and see if it works after you delete cache and cookies, and refresh the page.
If it still won't work, try the other CSS:
.wpdt-c .modal-backdrop{ z-index: 0 !important; }
If that still doesn't work, please provide me a temporary WP-admin login for your site where this happens, so we could log in and take a look ‘from the inside’ as that’s the most efficient way to see and resolve the issue. We do not interfere with any data or anything else except for the plugin (in case that’s a production version of the site), and of course we do not provide login data to third party. You can write credentials here just check Private Reply so nobody can see them except us.
input form doesn't appear in desktop size, but when I shrink the size of the window the input form appear .

Try to add one of the following codes in Custom CSS field in Custom JS and CSS tab of main settings of wpDataTables:
