This will be used internally by a clinic. The actual patient won't fill the form
I want to add the form on an internal page where only the employees of the clinic have access.
Once a employee gets a request for an appointment via phone, for example, they go in the site and fill out the amelia form.
How can I do this since once an employee is logged in and they go into the form, the first name and last name appears as the employees.
I want the first name, last name and email blank, even if someone is logged in in the site. Is this possible?

Sorry for late response we were on a conference meetings last week.
Unfortunately there is no internal page for this, because those fields like name, last name email will be automatically populated if you are logged in to the WordPress.
so maybe the workaround would be to have a page where your employees will access it, when they are not logged in to the WordPress and populate the fields for that customer, keep in mind that the first name, last name and email are mandatory fields and those fields will need to be populated.
