Yes, we have custom fields option with which you can create various types of custom fields that will be added to the booking form, and for each field you can choose for which service this question will appear. More about this option you can read here, and you can also try adding them on our demo site, just go to the Customize page and you will see the second tab Customfields.
what i meant is to ask questions in the same format like survey monkey where you press next and next ... untill you reach the page that starts asking your name ,click next , your number, click next, your email, click next, your postcode, click next, time of appointment.
