If I understand you right, you can make two categories and made for each category different service that you need. Like this you will be able to show on different pages each category separately. Please note that creating sub categories is not possible with built in features of plugin.

Sorry for misunderstanding,

We don't have page step by step for each case that customers request, so I will try to explain you as detail as I can.

To show tow different booking form you need to made more categories.

For free trial you will make categories Trials and then made service Free trials and set price to 0 and insert on page where your users can use it for free. You will use shortcode [ameliabooking category=1] or [ameliacatalog category=1] depends on what you fit better.

Then made another categories called for example Sign up and then made services called 3 month, 6 month, 12 months.... and set process for each service that you need and add it to this category. You will use shortcode [ameliabooking category=2] or [ameliacatalog category=2] depends on what you fit better.

Categories ID depends on how many you already made so you will insert right ones.

Like this you will have one form with free trial and another one with paid services.
