Please provide me a temporary WP-admin login for your site where this happens, so we could log in and take a look ‘from the inside’ as that’s the most efficient way to see and resolve the issue. We do not interfere with any data or anything else except for the plugin (in case that’s a production version of the site), and of course we do not provide login data to third party. You can write credentials here just check Private Reply so nobody can see them except us.

Can you please check if you maybe have some inline JS minifier tool enabled, e.g. W3TotalCache – inline minification of JS breaks the front-end uploader template logic.

Can you please try to deactivate wc-frontend-maganer plugin and tell me do you still have the same issue.
It looks like this plugin is throwing this error
Failed to load resource: net::ERR_CACHE_OPERATION_NOT_SUPPORTED
which may cause this problem.

I disable it but nothing happens. the same error.
I install also gravity form, and also with this plugin any user can upload the file.
