Unfortunately something like that is not possible out of the box, but in Amelia it is possible to export the appointments in the .csv file. If this software allow importing appointments from .csv file that you can try to do that.
You can try our sandbox site: Front-end & Back-end - you can find a fully functional version there to try out all plugin features.
So how we could export the .csv file? I don't see any option for that beside the manual export on the appointment section.
Is there any third party extension or plugin for Amelia what could extent the capability of this booking plugin?
Is there a custom CSS and JS option? I don't see any in the admin demo - although this could be done using the wp the theme custom css (divi in this case).
Also this plugin would need to be synchronized with the Carestream Dental Practice software so if there is a booking over a phone and entered into the Carestream Dental Practice software that needs to appear in this booking system to avoid double booking.

- Not for now. Front-end look of the Amelia pages can be changed through Customize page. For the future updates we plan to add hooks and filters in order to make Amelia customizable by the customers.

Well, so it seems like it is quite difficult to synchronize this plugin to anything, so it is a bit of pointless although the plugin is beautiful and we really do like it, it is outstanding compare to the 100 of appointment solution we have reviewed and I try my best to find a solution.

Dental softwares are running offline, not cloud based although we did create a little program for the carestream dental software which synchronize nicely with the current online appointment (https://www.appointmentor.com/) what we are using at this moment - trying to switch to Amelia now. Appointemntor is greatly outdated and not fully GDPR compliant so we want to move away from that.
Do you offer customization for your plugin, so you can create the connection using the api you mentioned for us?
Honestly I have never used Zapier. Probably it would be possible to connect your application with Amelia but that would require some custom coding.

If you want to allow them to see the backend of your WordPress, where they will be only available to see their own appointments, you would need to add them Amelia Customer role.
2. No at the moment. It is relatively new plugin and there are no add-ons on the market for Amelia. What would you like to extend?

2 more question:
We need to Register a new patient before that patient could book any appointment.
This is how we do it at this moment:

So existing patients can login and make any booking they like.
New patients would need to register first and pay the new patient registration fee before they can make any booking.
So there would be 2 option on our new site:
1) Register as new patient
2) Login for existing patients than make any booking.
How could we achieve this with your amelia plugin?
