So I started a second table manually. This time the multi-line-selectbox was not the problem. So I was wrong in that ticket. But I figured out a new problem:
usually you should 'apply' and 'safe' the new colum. If you forgot its gone and so far I didn't found out, how to add a new colum to an existing table when you made the mistake of having not apllied and safed the previous colums.
You can add columns in manual table as much as the limit how many columns is allowed per table in the mysql.(Approximately 4,096 columns per table) Can you check on your mysql server do you have some limitations?
Can you please check if there is a difference between adding a colum with and without "allow in-line adding". I switched off in-line adding and could add. With in-line adding I had all these reported probs.
If my hint is true then I would recommand to grey the green "add colum" button when 'allow in-line adding' is on.

Adding columns to the existing table is only possible in manually created tables or imported tables.

The link you send is the tutorial I know. My problem is something else:
1. I've a manually table and want to add a colum. See screenshots.
2. Apply and save
3. created colum is shown - but not registered in MySQL database. See attached screenshots 3 and 4
4. Where is my mistake? Seems its the prob to write to the database.

We tested that in our local environment and it is working fine adding columns in manual table with inline editing.

Did you made manual tables and add >50 column or you use making table form another data source?
If you made manual table can you check in your database in wp_options table option wdtGeneratedTablesCount is the same as the last made manual table?
